Probably not, but we are short on the number of indices: we hit a TeX
limitation for which I know no workaround.

I would suggest introducing a fake macro, something like

@c @hdrindex{MACRO}
@c ---------------
@c Indexing a header.
@macro hdrindex{macro}
@cindex @file{\macro\}
@c
@end macro
