There is AFAIK no standard but common practise is to use
column 32 for "normal entries" and column 48 for "individual
entries". The description should be a full sentence ending
with a period. Continuation lines are indented with an extra
space, for example,
* FOO: (FOODOC). Extensive documentation about the
FOO software package.
* FOOPROG: (FOODOC)FOOPROG invocation. FOOPROG entry.
