There is no reason to extract the pages from the pamphlet file.
Instead we modify the htadd function to use the pamphlet file directly.

With this patch bookvol7.1 replaces all of the .ht files with a
single file. The hyperdoc browser now fetches pages directly from
the pamphlet.
