The problem seems to be the include directive:
  include .//src/$(DEPDIR)/bar.Plo
in the generated Makefile; if I change it manually to:
  include ./src/$(DEPDIR)/bar.Plo
then heirloom-make works correctly.

Yes, the test passes by just adding the first line.

This one is sufficient o trigger the bug.  Here is a minimal testcase 
showing this:
  $ cat >good.mk <<EOF
  a:;
  include ./foo.mk
  EOF
  $ cat >bad.mk <<EOF
  a:;
  include .//foo.mk
  EOF
  $ : > foo.mk
  $ heirloom-make -f good.mk; echo $?
  0
  $ heirloom-make -f bad.mk; echo $?
  heirloom-make: fatal error: line 2: cannot get /foo.mk for including
  .
  1
