Try PS4=$'-$?\n+ ' for the `bash -x' case.  You can use PROMPT_COMMAND
for interactive shells.

The leading `-' is to reduce confusion when bash replicates the first
character of PS4 to indicate multiple levels of subshells.  Replace it
with any character you like.
