I suspect we're missing some fields... Back in the dim and distant
past, I think we came up with a list (well, many lists :-)) of header
fields. Which one is your form based on? 

Without looking back, I'd say, as a minimum we need:

1) a salutation field (Mr, Ms, Dr. Prof. etc
2) somewhere for those with letters after their name (I have a bronze
swimming certificate and I'm not afraid to use it :-)
3) a way of handling multiple authors
4) multiple middle names
5) space for the author to reference external (to GNE) peer review
results for this work e.g. to show it is an extract from an accepted
PhD thesis at university X.
6) e-mail address may need to be more than one field: one for
correspondence with GNE, and one for publication.

As a point of style, I'd also like to see password creation always
requesting it be typed twice e.g. the "desired password" field.
