off_t on MSVC

On MSVC 9, off_t is defined in <sys/types.h> and <wchar.h>. It is not
defined so far when <unistd.h> is included. But POSIX:2008 wants off_t to be
defined in each of

  <fcntl.h>
  <stdio.h>
  <sys/stat.h>
  <sys/types.h>
  <unistd.h>

This fixes it:

        Support for MSVC compiler: Ensure off_t gets defined.
        * lib/unistd.in.h: Include <sys/types.h>.
        * tests/test-fcntl-h.c: Check that off_t is defined.
        * tests/test-sys_stat.c: Likewise.
        * tests/test-sys_types.c: Likewise.
