Hi John,
It may well be a compilation or installation problem. I haven't checked
your file with groff 1.17.2 (which I don't have running here), but
with groff 1.16.1 on Linux I got no problems at all with your file;
it all came out looking as it should. The only minor point was a warning:

  macro warning: .NH 0 followed by .NH 2

But that's no problem since this was presumably what you intended
(or, even if it wasn't, it could have been ... ).
