Details:

Tables of contents in ms are hosed because they try to set an alternative
number format for the page number but this gets ignored, so the table of
contents shows up as another page "1".

I guess this has been broken for 20+ years.  Larry Kollar's ms.ms document
complains about it at the end--at least I _think_ this is what he was
referring to.

After much frustration today, I think I've fixed it.  Attached.

I'm seeing if it changes/regresses any of our other ms documents, and if it
doesn't, I'll write a regression test and commit.

Man, am I irritated!
