Well, your innocent example has uncovered bugs in 4 Programs!

. As mentioned above, lilypond should insert an entry for `.notdef'.

. While playing around with Type 42 fonts I found out that FontForge inserts
 two .notdef glyphs at position 0 in the CharStrings array.  While this is
 correct PostScript, FreeType doesn't like it, producing the abovementioned
 off-by-one error.  George has already fixed that; a fix in FreeType is
 not trivial, however (but this isn't an urgent issue since T42 fonts
 created by FontForge are quite rare).

. Finally, it shows a bug in gs itself -- version 8.15 renders OK, while
 8.55 fails.  Since I'm busy today I ask you to report this to the gs
 people.
