I don't know if this is related to the other extender problem I just
reported. It looks rather different to me (the lyric extender goes
too far, but doesn't go on forever).

Example:

\version "2.11.19"

<<
 \relative c'' {
   a2 b
 }
 \addlyrics { one __  }

% If this is changed to \addlyrics { one __ two } then the extender is short.

Sorry, I don't understand the problem -- what am I seeing in the example that I shouldn't be seeing? Please note that I know almost nothing about vocal music, so I have no clue how far the extender should go.

% I seem to remember that examples are supposed to be ragged right.
Is it possible
% to do this without writing explicit \score and \layout blocks?

Of course!  Just add
\paper { ragged-right = ##t}
