Not relevant to the immediate problem, but the
de-install left three files behind:
LilyPond/usr/bin/lily-pad-unicode.exe
LilyPond/usr/share/lilypond/current/dvips/ps/lilyponddefs.ps
LilyPond/usr/share/lilypond/current/dvips/ps/music-drawing-routines.ps
as well as the three files in the font cache.
I left these untouched and re-installed:
The first run accessed just the two smaller files
in the font cache, but still completed in less
than 5 secs. Subsequent runs less than 3 secs on
trivial files.

Exactly the same - all short compile times.
I de-installed again, this time deleting the
font cache, and re-installed without admin rights.
Now the first run takes c 1 min while it rebuilds
the font cache, but subsequent runs are the usual
3 sec. The font cache was rebuilt in exactly the
same place.
Tried running as a different user, one without
any priviledges and no admin rights. As expected,
the first run took 50 secs to build the cache for
this user - in [user]/.lilypond-fonts.cache-2 -
but subsequent runs took the usual 3 secs.
It seems my Vista runs LilyPond 2.11.42 perfectly.
I can't make it run slowly. So we need to find a
user with the problem to make any further progress.
Perhaps worth mentioning - my OS is Vista Home
Premium, fully patched up to today.
Is it possible to find out the differences between
the three versions of libfontconfig-1.dll? Might
this may have been upgraded for Vista, since the
later version fails on XP but works fine on Vista,
as the above demonstrates?
