On Mac OS X , the ~/.lilypond-fonts.cache-2 directory is created as
expected, but the font cache is not created if networked home folders are  
used.

This might be a Fontconfig issue, or an issue of LilyPond using deprecated
functions in libfontconfig that no longer produce the desired behavior.
