This is the tweak needed after the first 'r2' but before the 'c4.\( '

\once \override PhrasingSlur #'control-points = #'((1 . 2.5) (2 . 3) (21 .  
7.5) (47.3 . 2.85))

Attached is the PDF - I know we normally ask for png files but I think the  
PDF is more apt in this case so that someone else can check if the curve is  
smooth enough.
