Just to follow up on this...

After further testing, I'm almost 100% sure that this problem can be
solved by upgrading Pango to the latest stable release (in GUB).

Pango 1.26.0 has some major issues with shaping, notably for Khmer
script, and obviously for Arabic as well.

Graham, would you be willing to bump Pango in GUB?  I've been running
Pango 1.28.3 with my self-compiled LilyPond builds since October, and
I haven't noticed any issues -- only improvements.
