I didn't make any changes to MIDI, myself, but I have been watching, and the 
code controlling this behavior has not changed.

Oops.  That section of code didn't change, but the data streams that it work on 
did change.

It seems Javier found a new bug, because << {d'' b'} \\ {b' g'} >> sounded fine with 
version 2.12.3.  Added as <http://code.google.com/p/lilypond/issues/detail?id=1648>

  Which MIDI player do you use, Javier?
