Further updates.  A patch supplied from the texinfo community fixes this  
(see the end of this comment for the patch).  If this is applied to the  
currently delivered texinfo.tex, my tests show the problem fixed.  In the  
short term we should do this - I would like to push this change.  In the  
medium term, we should update the version of texinfo.tex to the current  
project version.  However, it appears that there is a bug in that version -  
it will not handle the line:

@funindex \relative

This is in the process of being reported.

As an aside, it's possible to test various versions of texinfo.tex in 2  
ways: 1) change the version present in the current working directory or 2)  
set an environment variable, like so:

TEXINPUTS=pathtotexinfo/::
export TEXINPUTS
texi2pdf testfile.texi

Note the :: after the /
