http://code.google.com/p/lilypond/issues/detail?id=1127This is a very helpful example, Keith. Either version 2 or 3 looks right to me, with 3 being preferable if space permits. Skyline-centred dynamics seem to be the norm in the scores I have perused this week, and they also have semantic value: keyboardists must sometimes distinguish between centred dynamics that apply to both hands or dynamics that only apply to one hand (or even to one voice within a hand). Ideally, LilyPond would apply the centred dynamics to both staves, but that probably belongs in another discussion. Thanks again!
