I did a very quick review of the code [1] for Lilypad on Windows. There is definitely logic in there for handling Unicode and UTF-8 encoding, and nothing stood out as obviously incorrect. I do not run the Windows build of LilyPond so I do not have Lilypad to try out.
[1]: https://github.com/gperciva/lilypadIt could be the version that shipped with 2.18.2 has problems. In particular, there were changes made only a few years ago to improve how LilyPad detects file encodings when opening files. Perhaps this is related.
