I have db 4.4.20 installed in the default location.

At the moment I have the development headers/libraries for db 4.3 installed via Debian/testing - that's installed in /usr/include:

/usr/include
/usr/include/db.h
/usr/include/db_185.h
/usr/share
/usr/share/doc

So I guess the question is: what is the "default location" where it
resides on your system?

make install put it at
/usr/local/BerkeleyDB.4.4/include/db.h
