This looks dodgey... the minor number on the left should
match the partition number.  i.e.  partition 6
should be /dev/hda6, etc.

I just fixed a bug recently that caused this problem, that
seems to only occur when you used RH 6.x's disk druid.

Try parted 1.4.23.
