I'd have to add more directives to do that...

As the author I feel it's useful to have an index that lists all the
directives and nothing but the directives.  It is not obvious from the
table of contents which directives are described where.

I realize this is a specialized situation, but trust me, this is the
most logical way to do this particular index.
