Small change for id's in <xref>

Hi to all:

     In the process of transforming from texinfo to DocBook using makeinfo
4.3, 
I realize that some id attributes of <xref> where not generated correctly 
because they have as a part of their names the characters $, ' and ¿.

     I also noticed that the attributes of some elements (such as
<enumeration>) were
not in lowercase, which will be helpful in case you like that the output
will be in 
DocBook xml and not in DocBook sgml.

     I made these changes to the file makeinfo/xml.c and I am sending you as
an attachment
the result of diff xml.c xmlchanged.c
