            In the documentation, it mentions that @smallexample,
    @smalllisp, @smalldisplay, and @smallformat should be used with
    @smallbook.  

Where does it say that?  I'm not sure that's necessarily true.

    This begs the question, why doesn't @smallbook
    automatically turn all calls of @foo into @smallfoo?

Because an author doesn't necessarily want all examples printed in a
smaller font.
