As it happens, Alper Ersoy just recently suggested another way to do
this, which has nothing to do with troff.

The idea is to insert terminal sequences (or markup that would be
translated into terminal sequences) into the Info output.  Then they
could rendered on consoles with underline or bold or whatever.  (Exactly
what would output as what is tbd, but titling commands like @heading
using standout mode, say, makes sense to me.)

We have been debating for several days on emacs-devel about the right
thing to do.
