You have to put any commands after @setfilename.  The only thing that
can occur before @setfilename is comments and \input texinfo.
At least I think that's it, I confess I didn't try it.

    I use makeinfo 4.5.93.

Was that what you were using when you looked at the XML output too?
You might try Texinfo from CVS.  As you may know, Alper has done a lot
of work with the XML/HTML/Docbook output in the current sources.  I'm
sure he'll answer in more detail when he can.
