Aha, sorry.

Again, I was supposing 7 bit CM fonts, with \v{C} as meant in good old
plain.tex .  This is good enough to print Dvo\v{r}\'ak in The TeXbook,
and it was used to typeset textbooks in ancient times, but it is not
``book quality.''   (In Czech, every second letter is accented.)

You have to use 8bit extensions of CM fonts for Czech book typesetting.
So it was again the same misunderstanding.
