I forgot to mention that this works, thanks.  Still a problem is the
insertion of blank lines for some commands in macros during expansion
in makeinfo.  For the moment, I've solved it like this:

  @defcodeindex ma

  @macro foo
  @maindex bar
  @c
  @end foo

but using address@hidden' is ugly.  I still don't understand why some commands
insert an additional newline...
