bad vertical alignment in index

Process groff.texinfo from the current CVS and look at the Concept
Index: A lot of entries are incorrectly formatted, this is, the second
column is moved to the right by one character.  This is quite ugly and
disturbs the eye heavily IMHO.  Below an example -- note the problems
in the entries for `.S' and `.tabs'.
