You need to have @setfilename.  It's required for making a nice output
filename (or processing with TeX, among other things).

    I have some questions about this output, divided into trivial and
    important.

See what texi2html's docbook output is like ...

    I am VERY new to Texinfo, so I'm probably making some huge mistakes.

In general, I don't have any answers for why the docbook output looks
like it does.  I just applied patches from people to support it
(originally in its SGML existence).  I know less about Docbook than you
about Texinfo :).  So if changes are needed, you are going to be the
expert recommending them.  If you can implement them, even better.

That's the relevant file in the C implementation, but forget about it
and work with CVS texi2html instead ...
