Regarding the index entry for @refill: I belatedly see the problem.  The
@findex refill
that was in the source was in an appendix that I had @ignored, and I
didn't notice the @ignore when I looked earlier today.  (Now I've
changed to using @c.)

I added @findex refill to the Command List :).  I hope this is the only
one that is missing, though I didn't exhaustively check, so it probably
isn't.  Maybe someday I'll extend the little
texinfo/doc/refcard/txicmdcheck script that I wrote to also check for
findex entries.

Sorry for missing the real problem there.  Sergey, you can forget about
that one.  But I think the unfound <N> problem is real.
