Follow-up Comment #2:

I have pushed an initial implementation.  Please give it a try and let me know
if there's something missing.  

Notice, that this implementation differs from its Emacs counterpart in that
the Up node of the virtual index window points to Top, instead of a temporary
node holding previously created indices.  Implementing such meta-index needs
more work because it implies some deeper changes to the structure of the
standalone info.  I'll address this later.
