The test...

# No index.html in ./t/results/converters_tests/sample_utf8/res_html
not ok 740 - sample_utf8 converted file_html
FAIL: t/converters_tests.t 740 - sample_utf8 converted file_html

fails on darwin because it is crafted requiring a case-sensitive filesystem
which isn't the default on darwin. Like wise it is impossible to check out the
texinfo 5.0 svn on a standard HFS volume due to the error...

This test should be re-factored to not require Index.html and index.html so
that it can be performed on non-case-sensitive filesystems.
