Thanks for the help.
I had an old texindex version. With latest texinfo 6.3, I get the same result
as you, if I do not use the OTF hack.
BTW, TexLive 2017 is out June 1, in less a week, so it probably has a later
LuaTeX.

Looks fine, except for that I also used:
@cindex 𝕥
@cindex 𝕗
and they do not show, as they are Unicode mathematical styles. The same problem
as before.
Also, perhaps these single characters should be assembled into one group in the
typeset index, instead of starting a new one.
Some books have a separate symbols index, sometimes more than one for different
symbol categories, sometimes listed in the order of appearance rather any
lexicographical order.
