Oops.  I figured it out.  I didn't notice earlier because I was just using grep to find the fnindex lines.

However, each of these @fnindex lines is inside @ifinfo/@end ifinfo.  That would explain it.

I couldn't reproduce this.  I checked with --no-split and without.

Could you try to produce a minimal failing example that you could
post to this list?

The code below has both bar and foo in the index.
