The following came up in the test suite changes:

  in&#160;@w&#160;<emphasis&#160;role="bold">b</emphasis><!-- /@w -->

i.e. the search and replace also took place inside tags, which is wrong.

I guess some kind of formatting stack is needed which is referred to
inside _protect_text or the conditional at the start of _convert handling
element text.
