I think I nearly have the code worked out to handle the
%contain_basic_inline_with_refs commands on the nesting stacks rather
than in _check_valid_nesting. Hopefully I can get the code for this
committed later today or tomorrow.
After that the next thing would be to integrate the 'regions stack'
into the nesting stacks too.
