As you say, it will only apply to very few manuals so I am happy for this
change to me made for the reasons you state.  Different capitalization in
different locales may not be an issue that will arise very much in practice
but it would be the best to define what happens clearly and this seems
the simplest to achieve with your suggestion, without wasting time on
obscure cases.
