lexical binding runtime

This is an implementation of the emacs runtime changes I talked about in
my earlier message.  Since the compiler doesn't know about them, the new
features have only been tested with hand-constructed byte-code, i.e.,
not much (but the impact on normal emacs usage is basically zero).
