the closest I've come across in  the emacs world is JDEE
management of  project specific settings, and I agree that emacs
could go a lot furher. 



I finally convinced myself to cut over to custom starting about 6
years ago,
and in the interim my .emacs has become more manageable --  I let
custom save its settings in a different .customie fle.
However the .custom file is now a dumping ground that brings back
nightmarish memories of the Windows registry
