Can we make the file header not part of the hunk either? Then the first `n' would move to the first hunk, and this should solve the remaining auto-refine problem.
To make things work like they do now, some (all?) commands that act on hunks would first see if point is inside a file header, and if so, jump forward to the next hunk.
