Summary: uploaded files do not show correctly when viewed

Initial Comment:
I have uploaded some text and images using the Docs and
Tasks.  I get no errors when I upload them but when I
go to view (before I approve), they have missing text
and images show up messed up.  I have wrote a script to
see if what is going on in gforge is correct (and test
my install of php) and I seem to get correct file
output, so I don't really know what to do.  Any ideas
on what is causing this?  My script takes a file,
addslash/stripslash/encode/decode just like the code
but it works here, so ...... HELP :) please.
