Changes since Fri, 26 Sep 2008:
* Prompt for buffer name when called interactively.
* Require buffer name when called programmatically.
* Set major mode according to default-major-mode.
* Preserve buffer-offer-save across major mode changes, unless its
permanent-local property is explicitly set (to any value).
