Word is that he's removing everything from within firmware/ on the next
release.

He doesn't.  Probably whoever split them out into the firmware/ tree
didn't even know about them at first.  The latest version claims they
were developed by the pcmcia-cs project.

Nope.  firmware/WHENCE only mentions GPL.

Nope, none of this.

A patch was posted that brought some sanity to the .cis files in Linux
last year (Sam mentioned it upthread), but it got nowhere.
