Extra requirements could have been added to fix that problem, but it's a
rare problem, and Apple's exploitation the GPL in this way proved
unsustainable.

So I'm not sure this problem merited adding any new requirements (and more
length and complexity) to GPLv3, in which case the problem will be no
smaller and no bigger under v3 than it was under v2.

But you said the problem has fixed itself already.  Is there an example of
the problem that still exists?
