American chords

American chords are still broken.  This was generated using 1.3.107,
although I suspect that it's still broken in 1.3.110.  Also, dvips is now
generating .ps files that cause ghostscript to barf (which doesn't happen
with lily 1.3.98).  YAP is able to handle the .dvi files though...
