I think in this case you can simply send a new patch that contains both
the changes of the previous patch you sent + the changes you are going
to make. Just make sure you mention it replaces the previous patch.
This is convenient both for you and for putting your patches in CVS.

Other options are: Copy your tree, make your changes only in one tree,
and then make a diff of the two trees by hand. Or you could use
"interdiff" (which can generate the diff you want from the previous
patch and the big patch including all changes), but I doubt this is
pre-installed on most system (I have it as part of a package called
"patchutil").
