[Gnumed-bugs] Possible failure of GNUmed to void duplicate values on a key

I mis-typed a name, and so ended up creating the same patient twice.

My problem is that I would have expected GNUmed to refuse creation of the 
patient using a duplicate value for

        Issuer / ID Type / Value

Is there a 'hole' in one of the constraints?
