No one's holding you back and nothing says there can't be both
even being used at the same time.

They're all in the same place - the edit area.

That's a data entry method. It says nothing about how things
are stored in the backend. The narrative data may still be scattered
among several tables (such as measurements.type = BP,
measurements.comment = "2 weeks ago at home"). This changes
nothing in the need for consolidation of narrative into as few
tables as makes sense.
