Sure, that's what I'm saying all along ! Look at
clin_item_type, lnk_type2item, v_pat_item_types and
v_types4item.

If we used columns in clin_narrative to store the code we
would either need arbitrary numbers of columns or use an
interpreted column which creates more trouble than it's worth
particularly since we are very likely to want to select by
given tags.
